Students attending University High School are eligible for a Carmel Clay Public Library card. We are asking all students in LSS to apply for the card so we can easily access audiobooks and other media for them, as needed. Please complete this form and return it to Ellyn Walerstein. You may scan and email the completed form or hand the completed paper form to Mrs. Walerstein.
